The Center for Implantology attaches great importance to your privacy. In this statement you will read about the data we process via this website, why we do so, and what rights you have.

This privacy statement applies only about the use of this website.
Uw behandeling, uw medisch dossier en alle gegevens die daarbij horen worden verwerkt door het St. Antonius Ziekenhuis, onder de Wet op de geneeskundige behandelingsovereenkomst (WGBO) en de Algemene verordening gegevensbescherming (AVG). Daarvoor geldt de privacyverklaring van het ziekenhuis: antoniusziekenhuis.nl/privacy-veiligheid. Medische dossiers worden op grond van de WGBO in beginsel twintig jaar bewaard.
We do not process any medical data through this website. It is deliberately geen open contactformulier op de site. Wel kunt u via het terugbelformulier vragen of wij u bellen (zie hieronder); daarin is geen ruimte voor medische informatie. Stuur ons ook geen medische informatie per e-mail — gewone e-mail is geen veilig kanaal voor gezondheidsgegevens.
3. What data do we process via the website?
Technical visit data (server log files). Our hosting provider automatically records each visit: your IP address, the time, the requested page, the referring page, and information about your browser and device. We use this solely to ensure the site functions properly, to resolve problems, and to prevent abuse and attacks.
- Foundation: legitimate interest (a secure, working website).
- Storage period: maximaal 30 dagen.
Terugbelverzoek via de website. Vraagt u via het terugbelformulier op de contactpagina of wij u terugbellen, dan verwerken wij uw naam, telefoonnummer, uw voorkeursvestiging en het moment waarop u het liefst gebeld wordt. Het formulier heeft bewust geen open tekstveld, zodat u geen medische informatie hoeft te delen. Uw verzoek wordt direct per e-mail doorgestuurd naar onze afdeling en wordt niet op de website opgeslagen. Wij gebruiken deze gegevens alleen om u terug te bellen. Daarnaast tellen wij anoniem hoeveel terugbelverzoeken er binnenkomen, met alleen de datum, de gekozen vestiging en het voorkeursmoment, zonder naam of telefoonnummer. Zo zien wij of het formulier goed werkt en hoe vaak het wordt gebruikt.
- Foundation: uw toestemming, die u geeft door het vakje bij het formulier aan te vinken. U kunt die toestemming altijd intrekken.
- Storage period: wij verwijderen het verzoek nadat wij u hebben teruggebeld, en uiterlijk binnen drie maanden.

What we don't do. We do not sell your data, do not use it for automated decision-making or profiling, and do not use it for advertising purposes.

Statistieken en advertenties. Deze website gebruikt geen statistiekdiensten (zoals Google Analytics), geen Google Tag Manager en geen advertentiepixels. Daarom ziet u ook geen cookiebanner. Gaan wij dit in de toekomst wel gebruiken, dan vragen wij eerst uw toestemming en passen wij deze verklaring aan.
5. Third-party services on this website
Google Maps. On our contact page, we display maps of our locations via an integration with Google Maps. As soon as that map loads, Google receives your IP address and information about your device, and Google can place cookies. The privacy policy of Google applies to this use: https://policies.google.com/privacy.
Google Fonts. Our fonts are loaded via Google's servers. Your IP address is also transmitted to Google in this case.
Hosting. Onze website wordt gehost door Hostinger, waarmee wij een verwerkersovereenkomst hebben gesloten.
Transfer outside the EEA. Google is based in the United States. Transfers take place in accordance with the EU-US Data Privacy Framework and/or standard contractual clauses of the European Commission.
6. With whom do we share your data
We only share your data if it is necessary or legally required:
- with the St. Antonius Hospital, insofar as it concerns your treatment;
- with your referring dentist, as part of the treatment and with your knowledge;
- with your health insurer, for claims and requests for authorizations;
- with our IT and hosting providers, solely as a processor and under a processor agreement;
- with authorities to whom we are required to provide data under the law.
7. Security
Wij nemen passende technische en organisatorische maatregelen om uw gegevens te beschermen, waaronder een versleutelde verbinding (HTTPS), toegangsbeperking op basis van rol, en up-to-date software. 8. Your rights
You have the right to:

- to have your data remove, insofar as there is no statutory retention obligation;

Een verzoek doet u via kaakchirurgiesecretariaat@antoniusziekenhuis.nl. Wij reageren binnen één maand. Om te voorkomen dat wij gegevens aan de verkeerde persoon verstrekken, kunnen wij u vragen zich te identificeren. Gaat uw verzoek over uw medisch dossier, dan verloopt dat via het St. Antonius Ziekenhuis.
